How to Obtain Transcripts and Certifications

The following information applies to students and alumni of the Programs in Audiology and Communication Sciences, Clinical Investigation, Genetic Epidemiology, Health Administration, Nurse Anesthesia, Occupational Therapy, Pediatric Nurse Practitioner, Physical Therapy, Psychiatric Epidemiology and the Schools of Dentistry and Medicine.

 In person, you may complete a transcript/certification request form in our office, located in Room 100 of the McDonnell Science Building. Office hours are from 8:30 am to 5 pm (Central Time Zone) Monday through Friday.

In writing, requests for transcripts, certifications and other items related to your academic records may be faxed or mailed to:

Please include the following in your request:

  • name (including name while in attendance if now different)
  • social security or student identification number
  • program and degree received
  • approximate dates of attendance
  • your address and phone number
  • address of the person to be sent the requested documents
  • sign and date the request indicating any deadline dates that may apply

N.B.: According to the 1974 Family Educational Rights and Privacy Act (FERPA), we must have your signed permission to issue your private record. Therefore, we must have a signed request from you and are unable to accept requests by telephone or email.

FEE

There is no charge for this service.

PROCESSING TIME

Please allow three to five days for processing your request, though requests are normally filled within 48 hours.

OVERNIGHT MAIL

To expedite delivery of your transcript or certification, please include a prepaid overnight envelope from an express mail service such as federal express or the U.S. Post Office. Please be certain that the service you choose will pick up at our office. Also, you should be aware that most overnight mail services will not deliver to a post office box. They encourage the listing of the building, suite number, and street address when delivering.
